



On May 13, 2026, PHMI successfully conducted a Root Cause Analysis (RCA) Training Program conducted by Consult Asia, Inc., which is designed to enhance the problem-solving capabilities of the participants and strengthen the organization’s continuous improvement efforts.
The training provided participants with a structured approach to identifying, analyzing, and addressing the underlying causes of operational issues rather than merely treating symptoms. Key topics included problem identification, data gathering and analysis, the 5 Whys Method, Fishbone Diagram, corrective and preventive actions, and effective implementation of sustainable solutions.
Through practical exercises, case studies, and collaborative discussions, participants gained valuable insights into applying RCA tools and methodologies in real workplace situations. The program reinforced the importance of fact-based decision-making, cross-functional collaboration, and systematic problem-solving in achieving operational excellence.
